EOM Sociodemographic Data Elements (SDE) Report

Sociodemographic Data Elements Report Workflow
1. Go to the Admin menu.
2. Select Reports.
3. Use the General menu list.
4. Select Patient List Report on the menu.
5. The report has multiple filters available.

6. If desired, select the Patient Status filter.
7. Select the desired population for the report. This is optional, especially if the practice is consistently using the EOM Chart Alert for EOM Beneficiaries.
8. If the practice is consistently using the EOM Chart Alert, select the Active Chart Alerts filter.

9. Check the box for Enhanced Oncology Model alert.
10. Once all desired filters are selected, click GENERATE. Depending on the volume of the report, it may take 2-5 minutes to generate.
11. Click on Generated Reports to retrieve the file.
12. Find and select the Patient List Report.
13. Click on the Download icon on the right-hand side.
The report will download in a CSV format, you may save the report to Excel for editing. Remove any columns containing information not required for the EOM SDE submission.

After setting the filters using the previous steps, click on SCHEDULE
In the New Schedule box, enter a name for the report.
Using the Frequency and Timing box, select the recurrence cadence, the day, and the time for future reports.
Select the date to begin the cadence of reports and, if desired an end date. It is recommended to check No end date.
Click NEXT.
Check the box next to Send it to my Generated Reports tab.
Click NEXT.

21. You may also have the report sent to other users’ Generated Reports tab by checking the box next to “Send it to others Generated Reports tab.” Start typing the name of the user.
22. In the Search Results box, click on the user from the list.
23. Clicking on the username will add them to the Selected Recipient box.
24. Click NEXT.
Review the Schedule Details.
26. Once you have confirmed the details, click SAVE.
