Order a Supportive Care Regimen

A chemotherapy regimen is ordered using Clear Value Plus's Pathway Decision Support tool. To order a Supportive Care regimen, you will bypass some of the prompts.

You start this process by going to the Orders > Regimens tab in your patients chart.

Select a Problem

In Diagnosis/Problem, choose the problem to be addressed by the injection. To add a problem that is not listed, click Other in the list. Begin typing the name of the problem. As you type, matching problems appear. Choose the problem.

Save a Problem

Click SAVE to add the problem to the chart.

Search for Regimens

Click SEARCH FOR REGIMENS to start the Clear Value Plus Pathway Decision Support tool.

Choose Regimen Type

To order a supportive care or non-chemotherapy regimen, like Iron Dextran, Hydration IV or a Vitamin B12 shot, click Supportive care Other for Regimen Type.

Filter Regimens

To find treatment options containing a word or phrase, type in the Filter Chemotherapies by field.

Click Select to make your selection.

View Regimen Details

In Patient Regimen Details, edit Regimen Name to reflect the treatment being ordered and how it should be named in the chart according to your practice's guidelines.

The default Cycle Length and # of Cycles are typically what is ordered. Common considerations for changing the Cycle are the patient's age and if the patient received previous cycles.

Each regimen is set to start on Cycle 1 Day 1. Consider changing this if the patient previously received this treatment.

To set the regimen Start Date to a day other than today, click the date.

Choose the Line of Therapy, if it is not already selected.

Click OK to begin editing the drugs.

Review Regimen Order

In the regimen drug list, to change the Quick Sig, click the Dose.

To change when the drug is given, click the Schedule.

To review your regimen order, click REVIEW / SAVE.

Order Review

Review your regimen order. To change anything about the regimen, click Regimens.

Select the regimen name to change only the Patient Regimen Details.

Click the drug name to change a specific drug.

If you are ready for the provider to sign the order, click SAVE ORDERS or SIGN ORDERS.

View Regimen on Flowsheet

The saved regimen is visible in the Flowsheet, but it is pending Insurance Authorization as indicated by Insurance in the Cycle/Day Header.

Document Injection in Supportive Care Regimen

When the Insurance Auth has been obtained, the regimen can be administered. To open the MAR from the Flowsheet, click the Cycle/Day header.

Medication Administration

Click Medication Administration.to take you to the Nursing Care > MAR.

MAR Documentation

On the MAR, enter the Incident to provider. Attending defaults to your patient's Attending physician, but change it if you need to.

You have three ways to enter the Given Time:

  1. To insert the current time, click now

  2. Use the time picker icon

  3. Type the hour (hh), the minute (mm), and choose AM or PM 

Documenting the Given Time also checks the Given checkbox.

To indicate a drug was not given, check HELD.

To record the Injection Location and other details about the administration, click Admin Details.

To record the drug administration and return to the Flowsheet, click SAVE.

View Given in the Flowsheet

In the Flowsheet, a G next to the drug dose indicates it was Given.
