EOM 4b: Plan of Care for Pain
Measure Description: Percentage of visits, for patients with a cancer diagnosis, currently receiving chemotherapy or radiation therapy who report pain intensity > 0 with a plan of care to address pain on the date of the qualifying encounter.
Relevance to Value Based Care: Proper pain management is critical to achieving pain control. Poor pain control denies patients comfort and affects their activities, motivation, interactions with family and friends, and quality of life. Inadequate cancer pain management is widely prevalent, harmful to the patient and costly.
Denominator:
All cancer patients, currently receiving chemotherapy or radiation therapy, who report having moderate to severe pain.
To be reported for each qualifying encounter where patient is documented as > 0.
Numerator:
Patients for whom a plan of care to address pain is documented on the date of the qualifying encounter.
Exclusions: None.
