Although Advance Care Planning (ACP) contributes to quality patient care, it has yet to become normalized in daily practice workflows. Some clinicians have a difficult time holding ACP conversations, delaying the ACP process, and resulting in patients’ end-of-life wishes and preferences not being heard or honored.

To reduce the Total Cost of Care by avoiding patients’ unwanted, prolonged life support
