Patient Submitted Response of PHQ Form
PHQ forms can be sent and received electronically from patients if your practice uses the Ontada Health Admin > Send Forms queue functionality.
When a patient fills out the Depression Screening (PHQ2/PHQ9) electronically, the results display in Patient History and as a PDF in the Documents tab. A number appears in a red circle next to Depression Screening to indicate there is a response from the patient to be addressed.
Based on the value, a task may be created in the Ontada Health Admin > General queue.
